  New Service for Patients

You may be aware that from April all practices are required to provide all their patients aged 75 and over with a named GP who will have overall responsibility for the care and support that our surgery provides to them. This does not prevent you from seeing any GP in the practice as you currently do.

 your NAMED GP will, however:

    1. Take lead responsibility for ensuring that all appropriate services required under the contract with the practice are delivered to you

    2. Where required, based on the professional judgement of the ‘named’ GP, work with relevant associated health and social care professionals to deliver a multidisciplinary care package that meets your needs
    3.  Ensure that your physical and psychological needs are recognised and responded to by the relevant clinicians in the practice

You do not need to take any further action, but if you have any questions, or wish to discuss this further with us, please contact us on 01706 860847
